Disguised as an abandoned factory north-east of the Raccoon Park, the P-12A Incinerator Facility, also known as the Dead Factory, was built by the Umbrella Corporation for the 'safe' disposal of Bio-Organic Weapons

, unwanted experiments, and general organic waste sent from local laboratories. Using a special type of gas to break down the organic matter of specimens dropped into a large dissolving pool within the facility, the dead factory is able to efficiently dispose of a large quantity of infected or dangerous material.

Unfortunately, the May 1998 tyrant virus outbreak caused by James Marcus in the Arklay Mountains caused increasing strain on the factory's systems. On July 16th, The plant's waste manager writes in his diary that he can't dispose of all the bodies, and that the "liquid medicine" was not good enough. On July 29th he also writes that the system function was decreasing and the number of bodies were completely swamping the system, the infection level was increasing and the anti-bodies were not matching the level of infection caused by the new mutation of the virus. Some of the workers were infected by the disease. More specimens were being delivered than the factory could safely dispose of, and it's highly likely that quantities of "t" escaped into the waterways and sewers that ran beneath the facility, contributing to the Raccoon City outbreak.

Sometime before October 1st and after July 29th. A US Special Forces team infilitrates the factory on orders to acquire a G-virus sample, they bring along an experimental anti-B.O.W. railgun, "Paracelsus' Sword". Five Tyrant T-103s were deployed to the factory via helicopter in order to destroy the Special forces team. In the resulting incursion all of the members of the US Team are killed as well as the 5 Tyrants.

During the events of October 1st, Jill Valentine, Carlos Oliveira and Nicholai Ginovaef found themselves within the facility attempting to acquire transport to escape the city. Unfortunately the Nemesis was following, after a battle within the facility's disposal chamber, the monster was dropped into the dissolving pool where Jill hoped it would be killed for good. However, the Nemesis survived and managed to crawl out of the noxious chemicals. No longer even remotely human in its appearance, it fed on a dead Tyrant and mutated into it's final form. It was finally terminated using the Rail Gun which had been installed in the factory's power room by an attacking government special forces team.

This facility was destroyed along with the rest of the city following the execution of Mission Code XX.
