The Fourth Survivor is a minigame found in Resident Evil 2 after completing both scenarios with an A ranking. Unlike most minigames, its events are canon; they follow the escape of Umbrella operative HUNK, now in possession of the G-Virus sample, from Raccoon City after his unit is attacked by the mutated William Birkin. The minigame's events are revisited in the chapter "The Fourth Survivor" in Resident Evil: The Umbrella Chronicles.

Players of The Fourth Survivor are tasked with guiding HUNK from the entrance to the sewers up to the Raccoon City Police Department's helipad, and eliminating or dodging all of the threats that stand in his way. HUNK's ammunition is limited, however, so running around most of the monsters will be required. The game is timed on how long the player takes to make it to the goal; the timer is displayed on-screen starting from the Dual Shock version of the game and in all subsequent ports.
