The Steam Pipe Maintenance Report, known as the Report on the steam pip failure (蒸気パイプ故障に関する報告書 Jōkipaipu koshō ni kansuru hōkoku-sho) in the original script, is a file that can be found in Resident Evil: Revelations.
Location
The file can be found in the engine room during the Episode 5-2.

To All Maintenance Staff
One of the covers came off of the pipes in the bilge passageway, and now there's steam pouring out of it.
The steam is scolding, so you'll have to do a little work around to get by it.
In the engine room there are two levers.
Use the levers to match the needle on the meter to the proper letters. This will stop the steam from spilling out of the pipe.
The letters and corresponding locations are as follows:
A: Maintenance Room
B: Engine room, upper level
C: Engine room, lower level
Remember if you stop the steam from leaking in one area, it will leak out of another.
Keep this in mind when adjusting the needle and using the relevant areas.
整備担当各位
船底通路内の一部の場所で、パイプ接合部の蓋が外れた事を原因に蒸気漏れが発生中です。
蒸気付近は強い熱を発しているので
次の方法で蒸気を調整し、通行して下さい。
動力室には２本のレバーが設置されています。
レバーを操作して 各アルファベットにメーターの針をあわせると、 該当の箇所の蒸気漏れを止められます。
アルファベットの対応箇所は以下
Ａ：機械整備室
Ｂ：動力室上階
Ｃ：動力室下階
尚、１ヶ所蒸気を止めるとそれ以外の箇所の蒸気は再度漏れた状態になります。
通行を行う際は、その点も留意して整備を進めて下さい。
